Follows American documentarian, River Rainbow Hagg as he travels to war-torn Syria, in hopes of documenting stories of the worst humanitarian crisis since WWII. He quickly realizes he has to do more than point his camera and uses his limited medical knowledge and joins a unit of foreign volunteers as they provide medical aid to civilians, YPG fighters, and ISIS.

The medical team is granted access to the front lines and they find themselves on dangerous ground, surrounded by ISIS. River hears bad news from home and plans his exit, but while crossing the border, finds himself in an Iraqi prison.
